Iron Infusion Dose Calculator
Ferric carboxymaltose (Ferinject and generics) for adults and adolescents 14 and over. Ganzoni deficit, the product information table and a dose plan.
For clinicians. Check the current product information and your local protocol before prescribing. Adult and adolescent (14+) dosing only. This doesn’t replace clinical judgement. Nothing you enter leaves this device.
Simplified table (body weight ≥35 kg)
- Hb under 100 g/L35 to under 70 kg: 1500 mg · 70 kg and over: 2000 mg
- Hb 100 to under 140 g/L35 to under 70 kg: 1000 mg · 70 kg and over: 1500 mg
- Hb 140 g/L or more500 mg (either weight). Recheck iron studies before any repeat dose.
Limits per administration
- Single dose: no more than 20 mg/kg, and no more than 1000 mg (20 mL).
- No more than 1000 mg per week. Give any further dose at least 7 days later.
- Under 35 kg: use Ganzoni only, with target Hb 130 g/L and stores 15 mg/kg (filled in automatically).
- Ganzoni rounding (PI): round down to the nearest 100 mg if 66 kg or less, round up if over 66 kg.
Safety
- Low phosphate. Common after ferric carboxymaltose, usually silent and short-lived. Repeat doses and risk factors (malnutrition, malabsorption, low vitamin D, hyperparathyroidism) can lead to osteomalacia and fractures. Check phosphate before repeat courses; ask about bone pain or tiredness.
- Skin staining. Extravasation can cause long-lasting brown staining. Stop the infusion at once if leakage occurs, and warn the patient beforehand.
- Hypersensitivity: give where resuscitation is available and observe for at least 30 minutes.
Alternative: ferric derisomaltose
Ferric derisomaltose (Monofer) is another high-dose IV iron. Trials show lower rates of low phosphate than ferric carboxymaltose, so it can suit patients at risk. Its doses and limits differ: use its own product information, not this calculator.
